Who makes them

Every template page is made by Lead Source, so it carries the company's name rather than a personal byline. The date beside it is the last time the page or its form changed.

How a template is built

The form comes first. We build the fields, then write the page around them: what each field is for, a filled-in sample, and the questions people ask about that form.

Each form lives in one source file. The page, the PDF in A4 and Letter, the Word file and the fill-online version are all made from it, so a field that appears on one appears on all of them, in the same order, with the same wording.

Who checks them

Templates for health, legal and money paperwork wait for a qualified reviewer before they are published, and the reviewer is named on the page. Templates that carry legal or clinical risk also say so on the form itself, in a note at the point where it matters.

How we use AI

We use AI tools to draft. Before a page is released it passes automated checks: every field against the form's source file, the copy against our style and claims rules, a uniqueness check against every other page on the site, and a check that every figure names its source. A page that fails any check is held back until it passes.

Keeping them current

Each template shows the date it was last updated. When the underlying form changes, the page, the files and that date change with it. The date never moves without a change.

What a template is not

A template is a starting point, not legal, medical or financial advice. Consent forms, waivers and anything that will be relied on in a dispute should be checked against the rules where you operate, by your professional body, your insurer or a lawyer, before you use them.

What happens to your answers

Filling a template online keeps your answers in your own browser. Nothing you type is sent to Lead Source or stored by us. If you ask us to email you a link back to a form, we receive your email address and the name of the template, and nothing else.

Spot a mistake?

Tell us through the contact page and name the template. We fix errors in the page and the files together, and update the review date when we do.
